I had heard about this secret level and that it should be unlocked when you finish the campaign. Well I finished the campaign and it wasn't there. I went searching and this is what I came up with.
It is even officially confirmed by Brian of Bungie.net...

There is a bug in the game. According to Bungie, all you have to do is finish Campaign on ANY difficulty level and you should get the new mp map. The problem is, if you did all of the on screen tutorials when they came up ("Press right and A to do a Barrel Roll"), you won't get it. You have to ignore one of the on screen tutorials, then do what you ignored on the last level. The barrel roll in the banshee is a perfect example.
If you already beat it and don't have (spoiler I guess) (which is a very Wizard-esque map), start a co-op game with your profile and a new profile (Easy mode is fine). Play as the new profile guy, leaving your main profile guy behind to just stand there. Get in the banshee and do a barrel roll...finish the game. The new profile guy will be able to select (map name)! So...delete the old profile and rename the new guy to your old name if you want.
Sounds stupid and it is...but it is the ONLY way to overcome the bug.
This MUST be stickied. Brian at Bungie talked me through this on the phone when I reviewed the game.

Shortcut If You Already Beat The Game
Credit to VJ1 on GameFAQs for this shortcut!!!
I'm talking to a guy that finished the game (I haven't yet, MP has taken over me) and he said that he loaded up the last stage on Easy with a NEW PROFILE, flew around with a Banshee until he got an on-screen tutorial (ANY tutorial counts, i.e. "Hold Y to dual-weild", you don't just have to wait for a Banshee help, ANY help screen will do), then beat the game...
To sum it up for those that didn't get it, if you didn't beat the game already (like me) just skip one of those on-screen tutorials, then try to get that EXACT tutorial done on the final stage...
For those who ALREADY beat the game and didn't get the map, just make a NEW PROFILE (might not make a difference), load up ONLY the last level in EASY in CO-OP with that extra Profile (to make it easier of course, I'm sure everyone played Normal first, unless you're a pansy) get an on-screen help message (easiest to try is a Banshee one for Barrel Roll I here, it happens the instant you fly off in it while fighting), do that tutorial, then beat the game... check your MP maps, and you should have a new map called:

There was no need to censor the name of the stage actually, but I felt that I should anyways for my fellow TXB'ers...
Yeah, it sucks that this happened, but hey, no game is perfect...
EDIT: Make sure that the character that is getting the Banshee e.t.c. is the NEW character profile, as the old one already got all of the help tutorials... if you're playing co-op on your own, just leave the old profile guy behind and go on and do the stuff/beat the game with the new character...
